Woolpit Health Centre is delighted to announce that we have recently appointed our own in-house Social Prescribing Link Worker – Cheryl Wiseman.

 

 

 

Cheryl has a passion for health and wellbeing and has worked in various roles relating to healthy lifestyle interventions.

In her spare time Cheryl enjoys family walks in the beautiful Suffolk countryside.

 

 

 

What is Social Prescribing?

Many things can affect your health and wellbeing.

Our GPs tell us that a lot of people visit them feeling isolated, lonely or anxious. They might be stressed out by work, money and housing problems and may need practical support. Sometimes it’s the stress of managing different long-term health conditions or needing lifestyle advice. That’s where Social Prescribing comes in.

It is a free service for anyone aged 18 years and over, registered at Woolpit Health Centre.

It starts with a conversation. Cheryl is here to listen, to give people time to focus on ‘what matters to them’ and take a holistic approach to people’s health and wellbeing. From here she can work with you to connect you to voluntary sector activities, support groups and community resources to promote health, well-being and independence.
